Important Tax Information.
Important Tax Information: Form 1095-C, which documents your health insurance
coverage for the 2025 tax year, is now available. Per the Paperwork Burden Reduction
Act HB 3797, employers are no longer required to automatically provide employees with
a paper copy of the employees 1095-B or C form. Employees can request a paper
statement if they choose. Please contact Jennifer Keilholz with the Osage County R1

provided within 30 days.
